By completing the Starlight Consent Form you have agreed to the following conditions.
You have agreed that Starlight Children's Foundation, its employees, officers, agents and contractors can take images or recordings of your child and/or family, whether by way of photograph, sound recording, video or otherwise.
You have agreed that the Starlight Children's Foundation, its employees, officers, agents and contractors can keep a record of information provided by you, your family or your child regarding your child's illness, medical story and participation in Starlight activities ("story").
You have agreed that the Starlight Children's Foundation, its employees, officers, agents and contractors can use, publish, reproduce, exhibit or distribute (in full or in part) the images and recordings of your child and your family and your child and family's story, for the following purposes: (a) the project/event described on the consent form; (b) internal Starlight Children's Foundation purposes. This is not limited to, but includes by way of example, internal presentations, circulars and posters; (c) external promotional, marketing, publicity and fundraising activities conducted by the Starlight Children's Foundation for the purpose of benefiting Starlight's not‑for‑profit and charitable goals. This is not limited to, but includes by way of example, use in newsletters, circulars, magazines, brochures, promotional and supporter emails, posters, advertising, the Starlight Website and the Starlight Annual Report; (d) use on Starlight's social media accounts and pages, including social media sites that are not hosted in Australia (and which are not regulated by or accountable under the Australian Privacy Act); and (e) retention and storage (including back-up) for the purpose of assisting Starlight to undertake the activities in this clause. This will include cloud services that are not hosted in Australia (and which are not regulated by or accountable under the Australian Privacy Act).
You have agreed that the Starlight Children's Foundation, its employees, officers, agents and contractors can distribute (in full or in part) the images and recordings of you child and/or family, together with your child and/or family's story, to third parties (including its corporate partners and supporters) for the purpose of those third parties engaging in fundraising activities for and promoting their association with the Starlight Children's Foundation, and you agree that the third parties can use, publish, reproduce, copy, exhibit or distribute these images, recordings and story for that purpose.
You have agreed that your first name and, where applicable, the first name of your child and/or other family members may be used in association with the images, recordings and story for the purposes set out in (2) to (4) above.
You agreed that no fee will be payable to you, your family (or your child) for the use of images, voice recordings, video footage or story as set out above.
